pHYSICAL EXAMS

Dear Timber Creek, Timberview and Trinity Springs Parents,

On Tuesday, April 21st, Timber Creek High School will host Physical Exams for student athletes to
fulfill the UIL Requirements for athletic participation for the 2026-2027 school year. This years
Falcon Feeder Physicals will be held in the Indoor practice facility on the north end of campus. For a
discounted price, student athletes will be evaluated by physicians we work closely with at Timber
Creek High School to ensure that their musculoskeletal system is cleared for athletic participation.
While this process will fulfill UIL physical and medical history forms, it may still be recommended
for your student athlete to follow up with their primary care physician for his/her Annual Wellness
Check.

Physical Exams will be available for all current 6th

– 11th grade student athletes within the Timber Creek
High School Feeder Pattern. They will begin at 3:00 pm and end at 6 pm. All student athletes MUST
complete a new medical history, online paperwork, and physical exam dated after January 1st 2026,
before participating in the 2026-27 school year.

It is important to note that if your student has any of the following, they will need to either complete
their physical with their primary care physician, or they will need to bring a clearance note from their
doctor for the physicians to review.

● History of heart murmur or any prior heart testing
● Any family members diagnosed with enlarged heart, dilated cardiomyopathy, Marfan’s
Syndrome, abnormal heart rhythm, etc.
● Currently under doctor’s care for recovering injury

Also they will need to complete the online athletic paperwork (kellerisd.rankonesport.com):

● Medical history must be completed, signed and dated by parent/guardian, and athlete.
● Ensure that all YES answers are explained in full detail on the medical history in the blank box.
● Fill out the top line of the Physical Examination Form (name, age, birthday, etc.).
 

Physicals will cost $15 dollars online or $20 at the door. Please be sure to have a card, cash, or check
ready during check in. Walk up entry will end at 5:30pm. We ask that you park in the Band parking lot
and enter the indoor facility via the Southeast corner garage door for check in.
